Microsoft has just been awarded a patent that was filed back in February 2006, where it outlines a computer based method for recording live audio and video, and monitoring a user's computer activity and tracking moments of attention loss.

The proposed recorder would make recommendations based on the moments your focus drifted off, and replace it with full A/V playback and the ability to hone in and amplify an individual speaker's voice.
